Good news!  Turns out I had another, apparently identical, card lurking
in my pile of Ethernets, and that one has the crystal solidly in place.
So I think I have a usable reference now.  Thank you all!

Based on the apparently-intact card, the crystal is placed with its
pins 1 and 2 away from the Ethernet jack (ie, towards the closest short
edge of the card).  Or, equivalently, pins 1 and 4 towards the PCI
connector, away from the closest long edge of the card.  The printing
is right-way-up when holding the card component side up, Ethernet jack
away from you (PCI connector on the left).
